OAVSNP Annual Awards

OVASNP Annual Awards, for the Oregon State Transition Conference, celebrate individuals that inspire, educate, and empower transition across Oregon!

Please nominate individuals using the Nomination Form that meet the following criteria for awards by January 31, 2019:

Matt Bailey Student Award

CRITERIA: Matt Bailey Student award recognizes an exceptional student with special learning needs who are involved in education or work experiences at the secondary or post-secondary level. Nominee should exemplify positive contributions in the workplace and/or community, showing growth and improvement, and demonstrates integrity and respect.

Outstanding Administrator Award

CRITERIA: Outstanding Administrator Award recognizes local school district administrators or regional/state administrators who made major contributions to the development and/or growth of successful outcomes for transition aged students or programs. Nominees should exemplify the best of the philosophy of education as it relates to supporting staff, transition aged students, and development of best practices and implementing predictors in post school success.

Innovative Agency Provider Award

CRITERIA: Innovative Agency Provider Award recognizes a non-classroom person (i.e. Transition Specialist, Job Placement Coordinator, etc.) who made major contributions to the development and/or growth of successful transition from school to employment. Nominees should exemplify the best of the philosophy of education as it relates to Innovation in successful placement from school to employment.

Extraordinary Support Staff Award

CRITERIA: Extraordinary Support Staff Award recognizes a classroom or non-classroom person (i.e. Special Education Assistant, Paraprofessional, etc) who made a major contribution to the development and/or growth of successful outcomes for transition aged students with disabilities. Nominee should exemplify the best of philosophy of education as it relates to successful transition from school to competitive integrated employment and that implements predictors of post school success.

Exceptional Program Award

CRITERIA: Exceptional Program Award recognizes an innovative transition program that has made a major contribution to the development and/or growth of vocational success for transition aged students. Nominee should exemplify the best of philosophy of education as it relates to successful transition from school to competitive integrated employment and that implements predictors of post school success.

Excellent Teacher Award

CRITERIA: Excellent Teacher Award recognizes a classroom teacher working with identified transition aged students with disabilities who made outstanding contributions in implementing predictors of post school success for students who are transition aged. Nominee should exemplify the best of the philosophy of education as it relates to the area of transition and post school success.

Stephanie Parrish Taylor Transition Rehabilitation Services Award

CRITERIA: Stephanie Parrish Taylor Transition Rehabilitation Service Award recognizes an Oregon Vocational Rehabilitation employee (I.e. VR Counselor, Human Services Assistant, etc.) who has made a major contribution to assisting students with disabilities to secure successful community integrated employment. Nominee should exemplify the best of the philosophy of Vocational Rehabilitation practices for transition aged students.

Partnership Award

CRITERIA: Partnership Award recognizes agency, business, or community partner who has made a major contribution in assisting students with disabilities to secure community integrated work-based learning or employment. Nominee should exemplify the best of the philosophy of inclusive business practices and integration for transition aged students.

“I Work We Succeed” Award for Excellence in Employment First Efforts

CRITERIA: “I Work We Succeed” Award for Excellence in Employment First Efforts recognizes a provider, agency, VR counselor, Educator, Service Coordinator or Personal Agent who has gone above and beyond to provide community employment services to transition-age youth. Nominee should be involved in Oregon's statewide efforts to support the Employment First Policy and should exemplify outstanding professional contributions and special efforts made by a staff member, provider or agency to help people with I/DD transition into community-based employment.

  8. I Work We Succeed –Excellence in Employment First efforts- Sandy Hyduchak from Lincoln Co. DDS
